Solution : Device IOS failed to initialize. Windows Protection Error.
Hi guys i have been this and other message boards and noticed that a lot of people still use windows 95 with new hardware and seem to have this problem.
As windows 95 is now unsupported by microsoft they wont tell you how to get your new hardware working with windows 95...Enter virtualdr. 
This is how you solve the problems some of these have been tried by other people and i have been told they work.
Solution 1
If you have a k6-2 or athlon processor.
You must underclock the cpu to under 350mhz im not quite sure how you would do this on a athlon(see suggestion 2 for another resolution) but on a k6-2 350 or higher consult you manual on how to under clock to processor, mainly it was jumper settings but in the later boards this was done in the bios
The settings i use is 3.0x100 make sure you note your settings before you do this.
now install the Amd K6-2 Patch which you can get from Here also some more details on how to get it to work. after the patch has been installed set your cpu to its previous settings (the ones i told you to note) and boot into windows.
Solution 2
Pentium 4 + possible athlon solution
Disable your l1 and l2 cache in the bios (consult your motherboard manual) boot into windows install the amdk6 patch Here re-enable the cache. and reboot.
maybe this could be made a sticky as like i said earlier i see this problem quite alot and people will open a new thread when the question has already been answered.
Any other solution /suggestions can be posted here.
