Security advisory MS03-026 was released on July 16, together with the patch. Anyone with Windows NT/2000/XP should have applied it by now, either through Windows Update (it should be listed as a critical update) or manually.
In practice, this worm is going to run for a while yet. Think back to the SQL Slammer worm. MS Blaster is a little slower to spread, but instead of infecting unpatched SQL servers it infects any unpatched Windows 2000/XP machine (workstation or server). The latter group is much larger in number than the former, and includes a large number of people who will get their machines compromised, reformat and go back to get infected again because they don't understand how they got attacked the first time.
