Defrag runs. Doesn't defrag....
Analysis said D: needed a defrag. I ran it. 20 minutes or so later it still had the same % fragmented....
Analysis said, you got it, You should defrag this drive.
What gives?!

Hey there, did the defrag program tell you that it was finished? If a defrag has not been done for awhile it usually takes much longer than 20 minutes. BJ

Another thing: Defrag is easily interrupted by XP continuing to write to the hard drive, which constantly restarts the procedure. Run Defrag in Safe mode for best (and least time-consuming) results.

Also, it may just be taking longer for this reason>>

No, it finished. Said it was done. I clicked to see report and it was the same % as before.
And I have not installed any 3rd party defrag...

If the drive is almost full or has really large files on it what you're seeing is pretty normal...If the drive is almost full you'll have to move some of the fiiles to another drive....
If you're happy and you know it......it's your meds.

After releasing some free space in your HD, it is possible that u should repeat the defrag several times, depending on the size of the disk, and the disk usage.
Given the topic, I have a question:
If u defrag rather frequently, even if XP doesn't prompt u to do so, is it bad for the disk?
Have a nice day

I wouldn't worry about it. You won't really see/feel/or taste anything different even if it does unfragment the files...
But, are there any applications installed on the drive that might be in use that would cause this?

Go to the Command Prompt (All Programs>Accessories), then type "chkdsk /r" without the quotes, type "Y" when asked if you would like to schedule the check for the next time that the computer is restarted, and then reboot the PC. That should repair any filesystem errors, and has been known to help with defrag oddities like this.
