I'm not sure there is one that is 100% spam proof.
I look for a provider nowadays that provides spam protection such as gmail (from google).
Any messages considered spam go straight into the spam folder. Yahoo do a similar thing as do Lycos.
Spam filtering is possible in Outlook but you have to state the rules for example any message that contains the word "viagra" can be automatically placed in deleted items.
The rules I created in outlook work fine, that's not my problem with it. The problem is, I keep it minimized and depend on the envelope in the systray to know when I have mail. But outlook shows it even when its junk, so I'm continually stopping what I'm doing and checking my mail, only to find out it's junk. Even adding "mark read" to the rule doesn't remove that env.
The answer to this one was another try at Thunderbird, properly configured.

Food for thought: I use dual monitors at the office and have Outlook Only on the second monitor. I can see at a glance if I need to check the mail.
I'm betting you're the only one in the whole co. that has 2 monitors, Steve. Thunderbird works great for my purpose now. The spam filter works both before you train it, and even better after, and it never notifies on junk arrival. That's all I wanted.
