If I add an external hard drive (usb 2.0), then will I have to format the hard drive or are they just plug in and become like another drive letter with immediate use. Is this the same circumstances for adding an internal drive?

Hard drives out of the box need to be partitioned and then formatted. There's a chance your external USB drive could be partitioned and formatted, but I doubt it since not everybody uses the same filesystem and cluster size.
yes you will plug it in and it will recognize it, but will not likely give it a drive letter until your format it and assign one threw computer management / disk management.
