Do outlook forms even work?

I'm trying to create a custom email form. You know, something that has a bunch of fields on it that someone can open and then email off to some destination for human review or automated processing.

I've looked at a few books and tried a few things and I'm really getting frustraited. This shouldn't be hard. I mean lotus notes is basicly all email forms - it's a very basic business function.

can anyone give me a short simple bullet list of the things I need to do in order to accomplish this?
