As I was afraid your hard drive went south:
Volume 1 C RAW Partition 146 GB Healthy
Volume 2 D RAW Partition 319 GB Healthy
The OS partition has become RAW. Never been able to convert a RAW partition to NTFS without losing the data.
If the problem isn't created by bad sectors on the HDD I think TestDisk can search for the lost NTFS partition and write a fixed one, it allow us to preview the files/folders inside before writing the new partition table, if the HDD is bad only commercial recovery programs can try to save some data for the readable sectors.

Hmm. I think the best thing would be to Reformat my PC.. Right? But what happened..?? I just did your instructions that's all.. Do you have any thought why this happened?

Reformatting won't do anything.
You need new hard drive.
Why did it happen?
I have no answer. Hard drives do fail.
