MalwareBytes(Free) & Windows Defender get along fine. Been using them several years now.
SpywareBlaster, gave up on several years ago.

What is a good, (preferred) free anti-spyware program that offers real-time protection?

I have no idea. I usually run mbam a couple times a week and let it go at that. Had no troubles in years.

BlackBeanie--MalwareBytes (paid) is an excellent real time protection anti-spyware program.
Spyware Terminator could also be considered.
However, Microsoft Security Essentials (free) should also be considered.

I am going to try Spyware Terminator - I guess I need to turn off Windows Defender, right?

1) Remove Windows Defender
2) Install all available Windows Updates
3) Download, install, update and run (scan with) Microsoft Security Essentials (Free)
4) Download, install, update and run (scan with) Malwarebytes Anti-Malware (Free)
5) Use Secunia Software Inspector to scan your system for other software that needs to be updated.

I think Windows Defender usually is automatically turned off by another antimalware program which conflicts.

For example, Microsoft Security Essentials disables Windows Defender.
