    DFT was run, no error in both Basic and Advanced Tests. Any suggestions?

    That's encouraging. It means (should mean) there isn't a hardware problem.

    I'd run the command:
    chkdsk x: /r
    next.

    (Note: Replace that x: with the letter of the drive that needs repair).

    That command tells chkdsk to attempt to fix any file-related errors on the disk and, if it locates any bad sectors, it will attempt to recover any readable information.

    I'm going to chkdsk soon. However, I don't want to take the hard drive out from the laptop again. Is there any way that I can creat a bootable CD and start the computer to DOS prompt with the problem HD connected inside. The I run the chkdsk there? Thanks.

    SpywareDr., Chkdsk was run. Still can't boot even in safe mode. This time, the computer failed when processing ntoskrnl.exe which is declared as missing or corrupted.

    I connected the drive via an external USB/SATA adapter to my laptop and went to the folders containing the data that I want to recover. Some of the files which couldn't be copied are now copied successfully. Most of these failed files disappear.

    Does anyone have any other suggestion before I format the HD for re-installation? Thanks a lot.

    Load XP's Recovery Console again. At the command prompt, type:
    Code:
    dir "c:\windows\driver cache\i386\sp?.cab"
    (and press [Enter]).

    You should see at least one sp?.cab file, (where the "?" is either a "1", "2" or "3"). For example:

     Volume in drive C is HD00_Vol00
     Volume Serial Number is B576-3C63
    
     Directory of c:\windows\driver cache\i386
    
    08/04/2004  05:00 AM        18,738,937 sp2.cab
    07/18/2008  10:37 PM        23,852,652 sp3.cab
                   2 File(s)     42,591,589 bytes
                   0 Dir(s)  180,557,402,112 bytes free
    Make note of the number in your latest sp?.cab file and use it in the following command, (in other words replace the "?" in the following command with either a "1", "2" or "3". In the example above, the number in the latest sp?.cab file would be a "3").

    Now type:
    Code:
    expand "c:\windows\driver cache\i386\sp?.cab" -F:ntoskrnl.exe c:\windows\system32
    (and press [Enter]). If you are prompted to overwrite the file, press [Y] (for yes) and then press [Enter].

    Now try rebooting and let us know what happens ...

    As I was creating the above response, found myself wondering if your system might have flaky/bad RAM? If it does, it could be tearing up all kinds of stuff on the hard drive. (RAM MUST be 100% accurate. 99.9999% = Junk). Might be a good idea to run either of the following overnight and see if you get any errors:

    Windows Memory Diagnostic
    http://oca.microsoft.com/en/windiag.asp
    (Burn the .iso file to a bootable CD and then boot the laptop from it).

    SpywareDr: Downloaded and ran the MS memory test. No error is found on standard and extended tests. I think the hardware (memory and hard drive) are OK. I'm going to format the hard drive and re-install the system by the recovery disk from Toshiba.

    I really want to find out or guess the cause of this problem. Any suggestions? Thanks.

    Dban the drive first with 3 layers. Anyway I would try it as the surface of the disk maybe corrupted.

    Spinrite ought to tell you if anything is wrong with the hard drive and aid in recovering corrupt data.

    Will Dban wipe out the whole hard drive or just the c: drive at my request?

    DBAN erases the partition table, which causes all drive letters to disappear.

    Thanks SpywareDr. I'm not going to run the Dban as the hard drive has a partition containin manufacturer's original drive image and other partitions with data there.
