A lot of them were caused by a bit of industrial espionage that went wrong. Somebody stole a formula for making the electrolyte, but they didn't realise that it wasn't the complete formula. The formula that they ended up using worked OK for a while, but did not age well, and led to all the problems. As jdc2000 said, in the latest equipment it is no longer an issue.
