Dr Watson error on explorer.exe

Ooops, WinNT did it again!

I got this error message from Dr Watson after loggin my NT:

Explorer.exe
Exception: Access Violation (0xc0000005),
Address: 0x715a5f94

and all icons disappeared.

Nothing appears on the screen even I click the right mouse (icons, taskbar have gone).

Please help me to get rid of this situation.
I can run any program via Task Manager except explorer.exe and Netscape.

WinNT, SP4

Now it was upgraded to SP6a.
Problems still there!!!!
